Submitting Your PhoneTree Pro Message Change Request

Please Note: The form below should only be used to submit message change requests for the PhoneTree Pro product. If you are using PhoneTree Patient Messaging, please contact Customer Support.

To our valued customers: Welcome and thank you for using PhoneTree to deliver your important messages. In the interest of processing your message change request as soon as possible, we have prepared this form to streamline the process. Please follow these instructions:

Upload your PhoneTree files.
(Instructions on how to upload your PhoneTree files will appear after clicking the Submit button, located at the bottom of this form. If you have already submitted this form and you need to upload files for a previous request, go here: http://64.132.143.37/promsgchange. If you have any issues with uploading your files, please contact Customer Support at 800.555.0559.)

Note: Your PhoneTree files MUST be uploaded within 72 hours of submitting this form or the request will be cancelled.

To add, remove, or block a provider (i.e., doctor, nurse, hygienist, etc.), please enter the name of the provider and the provider code (i.e., 12, JD, etc.) as it appears in your practice management software. For PhoneTree systems that support multiple languages, please specify any additional language(s) that are required (Note: PhoneTree offers studio recordings in English and Spanish only). For PhoneTree systems that support multiple practices, please list each practice the provider is associated with.

After submitting your request, please call our studio hotline at 877.886.8830 and leave us a message with the proper pronunciation of each provider name. This will help us provide you with accurate recordings.

To add, remove, or block a location, please enter the name of the location, the location code (i.e., 03, East, etc.) as it appears in your practice management software, and the phone number with area code for that location. For PhoneTree systems that support multiple languages, please specify any additional language(s) that are required (Note: PhoneTree offers studio recordings in English and Spanish only). If you would like a location's address mentioned in your message, please enter it along with any landmarks you use to help patients find your office (Example: "...at our East Office, located at 234 West Third Street, Downtown. Our office is directly across the street from the library.").

To add, remove, or block an appointment type or room number, please enter the name of the appointment type (i.e., New Patient, 45, etc.) and the corresponding code (i.e., NP, 45, etc.) as it appears in your practice management software. For PhoneTree systems that support multiple languages, please specify any additional language(s) that are required (Note: PhoneTree offers studio recordings in English and Spanish only). For PhoneTree systems that support multiple practices, please list each practice the appointment type is associated with. For appointment types that require special instructions to be given, please phrase them as you would to the patient (Example: "Since you are a new patient, we ask that you please arrive 15 minutes early to fill out necessary paperwork and bring a photo ID with you to your appointment.").
